Mission

Our mission is to provide dignity and enhance solace for families and individuals experiencing an end-of-life journey in a hospital or facing financial hardship due to serious illness.

What We Do

At Transitional Bridges we recognize people can feel isolated and scared when facing serious or life-threatening illness. That is why we partner with facilities to share our compassion kits and offer patient assistance grants to improve quality of life during health crises. We believe everyone deserves dignity and respect during their most difficult times.
