What we need
- A professionally designed online survey ready to use (set up with Google Forms, SurveyMonkey, Typeform, SurveyLegend, or other tool)
- Training to equip staff to execute and manage survey
Additional details
We are looking for someone to set up a short, five question survey/application form for prospective grassroots partner organizations. We are partial to Google Forms and ask that the survey/application please be configured for that platform.
What we have in place
- We currently have a rubric that you can use to develop the questions.
How this will help
This project will save us $4,828 , allowing us to devote more resources to our work with grassroots organizations in the field.
The development of this survey will allow us to streamline our application process for re-grants to partner organizations and will ensure that we can work with aligned organizations whose work accords with our eligibility requirements for partners.

Our mission
Caring Across Generations ("Caring Across") is a cross-sectoral campaign of family caregivers, care workers, people with disabilities and older Americans coming together to create a nation where everyone can live, work and age with dignity. We are dedicated to the vision of nation that provides universal long-term services and supports (LTSS), childcare, and paid family and medical leave (PFML) benefits; encompasses a supported and powerful nation of caregivers; and develops a culture in which care is valued, visible and collective.
