What we need
  • An easily understood visual aid that communicates complex information
  • A high-resolution version of the Infographic in the Organization’s preferred format

We are creating an infographic for an academic poster to demonstrate our community pharmacy program's workflow. we need a simple and clear flow chart that can be used to show the steps our team takes to ensure out unhoused participants get their medication in a timely manner

What we have in place
  • We currently have a rough draft, which should make it easy for you to get started. We also have colors and themes, and the ability to provide any other information you need.
How this will help
This project will save us $4,212 , allowing us to treat more people!

This will help other pharmacy groups sharpen their ability to serve unhoused patients

  • Volunteer Manager and Professional discuss the objective of the infographic, key audiences, and style preferences (if any)
  • Volunteer Manager shares examples of infographics they like with the Professional
  • Volunteer Manager provides Professional with data or other source of information to draw from

Milestone 1: Preliminary Sketching
  • Professional develops three drafted concepts to share with the Organization
  • Volunteer Manager chooses one draft as a primary concept and provides feedback

Milestone 2: Graphic Creation
  • Professional provides Volunteer Manager with first draft of Infographic
  • Volunteer Manager provides Professional with up to two rounds feedback

Milestone 3: Presentation of Deliverables
  • Professional delivers the final version of the Infographic including specifications for usage (colors, size, etc.)

Our mission

End Hep C SF envisions a San Francisco where hepatitis C is no longer a public health threat, and hepatitis C related health inequities have been eliminated. Our mission is to support all San Franciscans living with and at risk for hepatitis C to maximize their health and wellness. We achieve this through prevention, education, testing, treatment, and linkage to reduce morbidity and mortality related to hepatitis C. We believe that everyone should have equal access to prevention and care regardless of individual characteristics.

What we do

Provide interventions that are evidence based, and continuously review our progress to determine areas where we need to improve, through the regular collection of and use of local data related to hepatitis C.
Work to end stigma about hepatitis C and people living with hepatitis C.
Maximize the health and wellness of people who use drugs by treating them with respect, ensuring access to appropriate services, and empowering them to reduce harm and make choices to improve their health.
Continue to invest in populations that have frequently been characterized as "difficult to engage," as we realize that these groups often have the greatest unmet need for services and support.
