End chronic homelessness in Santa Clara County.

What We Do

Destination: Home is a public-private partnership serving as the backbone organization for collective impact strategies to reduce chronic homelessness in Santa Clara County. In this role, Destination: Home drives and aligns resources to end chronic homeless and is improving how systems work together to intervene with episodic and situational homeless individuals and families at risk of becoming chronically homeless.
