What we need
- Analysis of the current process and creation of a current state map
- A document outlining potential goals of process improvement

We would like to replace a piecemeal system built on an inefficient, static database with an efficient workflow, better working interfaces for staff, and more nimble reporting (ability to add new assessment or new report) but have to keep populating the old database with some of the data.
What we have in place
- We currently have Lucid chart workflow, Microsoft 365, which should make it easy for you to get started. We also have some technology background, and the ability to provide any other information you need.
How this will help
This project will save us $6,868 , allowing us to upgrade a badly outdated technology workflow, which might not be affordable otherwise.
The program is a state funded program to support low-income, older adult clients to avoid nursing home placement by safely living at home. An efficient workflow will prevent case manager errors and save time so they can help their clients more effectively. This will increase the value of the tax dollar investment, which benefits the greater community. It could also lead to stabilizing the clientele and decreasing staff turnover.

Through a comprehensive network of resources, Sourcewise strives to educate, prepare, support, and advocate for all adults, their families, and their caregivers within Santa Clara County.
What we do
We work to coordinate services and support systems for the aging population across a network of state and local organizations in Santa Clara County and help empower seniors and caregivers by providing access to information and resources, and supportive services allowing for personal choices, and continued independence, so they may remain safely in their homes.
