What we need
  • Design of one template for a document based on Organization's Visual Brand Guidelines that can be customized for various needs (e.g., letterhead)
  • Design of one specific document (e.g., appeal letter)
  • Note: This project is for a single page template only. For longer presentations, please use the Presentation Creation project
Additional details

1. we need 2 different documents designed into help sheets for staff/volunteers
2. we need stickers designed to use as labels on clipboards
3.pick up results reminder designed

What we have in place
  • We currently have an outline with final content, which should make it easy for you to get started. We also have logos and branding details, and the ability to provide any other information you need.
How this will help
This project will save us $945 , allowing us to purchase incentives for patients

This is helps keep staff and volunteers organized while providing services for patients

Our mission

To improve liver health for all by providing mobile liver health screenings and linkage to care.

What we do

The DeLIVER Care Van is a mobile hepatitis C clinic. We offer every service along the hepatitis C care cascade from screening to treatment. We are a new program and have been on the streets offering our services to San Francisco since January 2019. We first started as only a mobile testing unit, but in May (only 5 months after being operational) we were able to start offering hepatitis C treatment on the van. We are a one-stop shop for folks curious about their hepatitis C status and for folks interested in getting their hepatitis C cured.
