Additional details
We produce consumer guides on navigating hospice and palliative care services in Minnesota. We used Tembua to translate these guides in Spanish, Somali, and Hmong. Now we would like them to be edited one final time to ensure our message is conveyed correctly.
We need someone that can edit in Somali to ensure the message in the English consumer guide is conveyed properly.
What we have in place
- We currently have the English and Somali consumer guides, which should make it easy for you to get started. We also have availability for phone meetings, and the ability to provide any other information you need.
How this will help
This project will save us $7,098 , allowing us to further our mission of allowing people agency in end of life, and making hospice and palliative care more accessible to the diverse communities Minnesota is proud to have.
This organization will help us reach the diverse communities in Minnesota. Hospice and Palliative care are used by almost everyone, so they should be accessible to everyone. It is important to have the guides available because many people enter into hospice too late. Discussing hospice can also be an uncomfortable and stressful time for families, so having resources to guide their conversation in their primary language is crucial to expanding hospice care.

We strive to ensure that everyone has the help they need to live and die according to their wishes. We do this through education and advocacy around serious illness and end-of-life care.
What we do
Minnesota Network of Hospice & Palliative Care is an organization made up of health care providers, individuals, and allied organizations all committed to advancing access to, knowledge about, and quality of hospice and palliative care in Minnesota.
We are a resource for individuals and families who are coping with a serious illness or are facing the end of life. We provide information to the public about hospice and palliative care and help connect individuals and their loved ones with the are they need.
We are also a member organization for health care organizations and individuals in the hospice and palliative care community. We provide opportunities for education, including webinars, our annual fall forum, workshops, and our annual conference, which welcomes more than 1,000 people and is the largest conference in the region focusing on hospice and palliative care.
We collaborate with regional organizations and national leaders to provide a state-wide perspective on advance care planning, palliative care, and hospice care. And we advocate for greater awareness about and access to hospice and palliative care.
