This brochure is for our Transition to Wellness Medical Respite program. People exiting acute hospital care without stable housing are served by this program so they can regain health with a roof over their heads with compassionate staff to help them stabilize. This brochure is circulated to social workers at local hospitals and skilled nursing facilities, who refer clients experiencing homelessness to our program. Founded in 1974, Homeward Bound has grown to become the main provider of shelter and supportive housing for vulnerable people experiencing homelessness in Marin County, California, including working families, transition age youth, veterans, seniors, and individuals with incarceration histories, disabilities, or mental health issues.
Backed by a mission of "opening doors to safety, dignity, hope, and independence," our ultimate goal is to end homelessness for everyone we serve.
