Mission

Reliance Community Care Partners is an independent community case management organization that works with the under-served Medicaid population. Our programs are funded either by the State of Michigan or local community funding.

Our mission is to promote the dignity of the individuals we serve by coordinating and providing quality services that focus on education, choice and advocacy.
