We believe community health workers are the frontline of defense against a national epidemic of preventable chronic diseases.

What We Do

Founded to support the increasingly expensive and overburdened health care system, City Health Works hires and trains clinician supervised Community Health Workers from the neighborhoods we serve to act as Health Coaches.

Coaches motivate individuals to achieve realistic health goals through a holistic approach:

• Provide personalized motivational coaching, coping strategies and health education in community or home settings to improve self-management of chronic diseases and mild depression.

• Identify & communicate signs of medical complications to the individual's doctor to direct patients to the right care at the right time.

• Connect members to local services to address socio-economic challenges.

• Train “social champions” from the member’s household to provide long-term social support.
