Child Health Partnership supports families with children age 0-6 to build nurturing home environments and promote the health and wellness of young children in our community.

What We Do

Child Health Partnership is a home visiting program that offers teams of nurses and family support specialists to work with families and pregnant moms to support child health, positive parenting and family self-sufficiency.
