Mission

BCHN is a community-based organization and Federally Qualified Health Center dedicated to improving the health and well-being of Bronx residents, especially the uninsured and medically underserved.

What We Do

We facilitate access to affordable, quality primary, behavioral and oral health care, diagnostic, pharmacy and social support services for 100,000+patients annually at 15 Bronx community and school-based health centers. Public and private grants help to underwrite the cost of care for approximately 20,000 under- and uninsured individuals. Our health educators, dietitians, patient navigators and ER liaisons educate thousands of patients and community about chronic disease risk factors, nutrition and fitness options; assist with public health insurance, including Affordable Care Act health plans. They save almost a million dollars annually by helping patients to reduce non-urgent ER use. With our state and national associations we advocate for services and funding that meet community needs; make appointments and connect over 2500 individuals with a personal doctor annually; leverage resources through partnerships and collaborations; monitor health center services to ensure compliance with quality, financial and administrative standards.
